Preceptor Home Health & Hospice cares for patients across Southeast Wisconsin. Offering the highest quality of care, our multidisciplinary team members develop a care plan based on the individual needs of each patient. Team members are including but not limited to: Physician, Nursing, and CNA Care, Social Work, Chaplain Services, Volunteer Services, and more.

Team members at Preceptor focus on the holistic hospice approach in addition to physical. social, emotional, and spiritual aspects of care. Our Hospice Volunteers are essential in supporting Preceptor's mission of promoting the comfort and quality of life of each individual patient.
