The Families + Neighbors Together (FNT) and Student Engagement Team (SET) program are community-based Sheboygan County Health and Human Services programs. 


The FNT program highlights our commitment to our community and residents. The work is centered in two City of Sheboygan neighborhoods where additional support is needed. The south side location is the Wesley House, located at 829 Union Avenue. The SET program is also located in this south side location. Our north side location is at St. John's UCC, located at 1248 Lincoln Avenue. 

FNT is community-focused and engaging to local stakeholders, not-for-profit agencies, schools and, most importantly, residents of these neighborhoods to create collaborative networks.  These networks focus on neighborhood-based prevention and early intervention efforts to support families, communities, and agency partners. This program offers resource referrals, advocacy support, and problem-solving assistance to residents in the areas of housing, food security, employment, and health via relationships.

The SET program is a voluntary program working with students at Sheboygan Area School District Middle Schools (Etude, Farnsworth, George Warriner,  Horace Mann, and Urban) , Howards Grove Middle School, and Random Lake Middle School. Students are referred by school social workers or other identified school staff. 

SET provides the student, their family, and the school district with an option besides the traditional responses of using a citation or making a referral to Youth Justice to address truancy. SET involves both the student and their family participating in agreed-upon supports and services based on their needs. It is the intention of SET to provide support and assistance so that students can be successful in school, at home, and in the community. 

The Wesley House location is also in the process of starting a community garden for FNT/SET families and neighborhood residents to utilize. This can be an avenue to build community connections amongst neighbors and increase access to fresh produce.. 

Details: 

Clients in both the FNT and SET programs are frequently in need of basic household items or food. Current needed items can be located on our Amazon Wishlist. Items donated will directly benefit clients in the FNT and SET programs. No donated items will benefit Health and Human Services employees.
